Cross Connection Survey Report

Cross Connection Surveys Report - How to Guide

Have you received a letter from the Town of Markham, Toronto, Peel or another municipalty asking for you to submit a Cross Connection Survey Report, here is what you need to know?

What is a Cross Connection?

A cross connection is any physical connection or arrangement between two otherwise separate piping systems, one of which contains potable/drinking water and the other either water of unknown or questionable safety.  For example, a boiler or sprinkler system connected to a potable water system.

What is a Cross-Connect Survey Report?

The City of Markham requires property owners to have a qualified person to identify the risks they have for cross connections. The qualified person will identify and record any potential cross connection hazards inside the property that is not properly protected. The survey will track all cross connections found within the property.  The survey will also identify any existing protective devices already installed indicating the serial number and last test date.  The survey will indicate if the existing protection is acceptable or provide recommendations for protection.

Who is Qualified to Complete a Cross Connection Survey?

The City requires a qualified licensed plumber with O.W.W.A certification to complete the cross connection survey.  Markham Backflow guarantees a licensed plumber with full O.W.W.A certification will complete the survey.

Other Backflow and Survey Information

The qualified and licensed plumber will document the following information on the cross-connection survey report.

  • Facility information, type of water use, hazard level

  • Qualified person (O.W.W.A cert number)  and facility person contact information

  • List of backflow prevention types and associated codes

  • List of any cross connections including their location, hazard level, and if there is any existing protection device

  • Information of testable backflow devices, including serial numbers and last test dates

  • Comments sections for additional information for each cross connection

  • Signatures of the plumber and facility contact
